General Background: Third-party logistics providers require reliable warehouse and distribution processes to control product losses, inventory discrepancies, picking errors, and shipment delays. Specific Background: Home delivery operations at PT DSV Solutions Indonesia encompass receiving, shared storage, inventory management, order picking, packaging, and distribution, with 168 product units reported missing during 2025. Knowledge Gap: Integrated applications of Failure Mode and Effect Analysis and House of Risk remain limited in comprehensive third-party logistics operations using shared storage systems. Aims: This study identified operational risk events and agents, established treatment priorities, and formulated preventive strategies for fast-moving nutrition and supplement products. Results: Failure Mode and Effect Analysis ranked identified failures through Risk Priority Numbers, while the House of Risk framework identified 32 risk agents. Nineteen priority agents represented 80.35% of total Aggregate Risk Potential. Implementing the Jolly Box system and Stock Keeping Unit labeling achieved the highest Effectiveness-to-Difficulty ratio, with an ETDk value of 4,037. Novelty: The study integrates two risk assessment methods across the complete home delivery workflow within a shared-storage logistics setting. Implications: Structured containers, clearer product identification, and priority-based preventive actions can support inventory accuracy, warehouse control, and service reliability.
