General Background Employee health services are important for supporting workplace welfare, productivity, and sustainable human resource management. Specific Background The medical clinic in a tax office setting provides internal health services, yet field conditions show that not all employees comply with prescribed medication therapy. Knowledge Gap Limited attention has been given to how health education relates to medication adherence through health literacy and self-care among employees receiving internal clinic services. Aims This study examines the relationship between health education and medication adherence, including the mediating contribution of health literacy and self-care. Results The study involved 107 employees using total sampling and Structural Equation Modelling with Partial Least Squares. All tested direct relationships were significant: health education was related to medication adherence, health literacy, and self-care; health literacy was related to medication adherence; and self-care was related to medication adherence. Health literacy significantly mediated the relationship between health education and medication adherence with T statistics of 2.191 and P value of 0.028. Self-care also significantly mediated the relationship with T statistics of 2.016 and P value of 0.044. Novelty The study positions health literacy and self-care as behavioral pathways linking health education with medication adherence. Implications Internal clinic education should be combined with literacy strengthening and self-care practices to support more consistent medication-taking behavior.
